LIABILITY WAIVER

No signature required. Submission of your name and email serves as agreement to this waiver.

By providing your name and email address, you acknowledge and agree to the following:

1. Voluntary Participation: I am voluntarily participating in cold plunge and wellness activities, and I understand the nature of the services provided, including exposure to extreme temperatures.

2. Health Disclosure: I certify that I am physically able to participate and have no medical conditions or concerns that would prevent me from safely using the cold plunge or participating in wellness services.

3. Assumption of Risk: I understand that cold exposure may involve risks including, but not limited to, changes in heart rate, blood pressure, dizziness, fainting, and muscle soreness. I assume full responsibility for any injuries or health-related issues that may arise from participation.

4. Liability Release: I release and discharge Melt Well Sauna + Plunge Studio, its owners, employees, and affiliates from any and all liability, claims, or demands for injury, illness, damage, or loss arising from participation in this event.

5. Photography Consent: I grant Melt Well permission to use event photography or video for marketing and promotional purposes, unless I opt out by notifying staff.

6. Minors: If entering information on behalf of a minor, I certify that I am the legal guardian and accept full responsibility under this waiver.
