LIABILITY WAIVER

I have read and agree to the Liability Waiver below. By submitting this form electronically, I acknowledge that my submission constitutes my electronic signature and agreement to this Liability Waiver. I also agree that Melt Well may contact me by email and text regarding this giveaway, wellness information, future events, and promotional offers. By providing my information and submitting this form, I acknowledge and agree to the following:

1. Voluntary Participation: I am voluntarily participating in cold plunge, infrared sauna, contrast therapy, red light therapy, and related wellness activities. I understand the nature of these services, including exposure to extreme hot and cold temperatures.

2. Health Disclosure: I certify that I am physically able to participate and have no medical conditions or concerns that would prevent me from safely participating. I understand these services may not be appropriate for individuals with certain medical conditions, including cardiovascular disease, uncontrolled high blood pressure, pregnancy, seizure disorders, recent surgery, or other conditions where exposure to heat or cold may present increased risk.

3. Assumption of Risk: I understand that participation in cold plunge, infrared sauna, contrast therapy, and red light therapy involves inherent risks, including but not limited to changes in heart rate or blood pressure, dizziness, fainting, dehydration, burns, slips and falls, muscle soreness, skin irritation, allergic reactions, aggravation of existing medical conditions, serious injury, or other unforeseen risks. I voluntarily assume full responsibility for all risks associated with my participation.

4. Liability Release: To the fullest extent permitted by law, I release, waive, discharge, and hold harmless Melt Well Sauna + Plunge Studio, its owners, employees, contractors, agents, affiliates, and representatives from any and all claims, liabilities, damages, demands, actions, or causes of action arising out of or related to my participation in this event or any associated wellness services. This release applies to the fullest extent permitted by applicable law.

5. Photography & Video: I understand that photographs and video may be taken during this event and may be used by Melt Well for social media, marketing, and promotional purposes. If I prefer not to appear in photos or videos, I will notify a Melt Well team member before participating.

6. Minors: If I am completing this form on behalf of a minor, I certify that I am the minor's parent or legal guardian and agree to this waiver on the minor's behalf.
